Volunteer Application

This application is one of the documents to be completed by all volunteers within our church who will be working with children/young people (0-18 years). A separate form is required for each ministry in which a person is seeking to serve.

Completed forms will be retained indefinitely as part of the parish's Safe Ministry records.
